Embrace the Suck; Panama is Preparing to Be a Covid-19 Vaccine Distribution Center; Covid-19 Free Taboga; Draft Law 354 on employment protection; Wise Words

Thursday, July 30, 2020

This sucks. There’s no doubt about it. The worst part of this suckiness is that it’s the type of suck that we can’t readily control. Today I had a talk with a friend who’s in a position where he has to abandon ship; time to close up shop and head back to the ‘States. Because of that, there are several other related businesses that are going to have to come to a halt as well. This is no hit on him; he’s just become an integral part of the suck. He’s become an unwilling participant of the standard suckitisation. In midstream of our talk, I told him that I was going to write about Controlling the Suck, but as I just said, we can’t control it. (Or can we?) Our conversation moved to, “Why did this happen? Why does this suck?”. Whether, this pandemic was initiated purposefully by a group of countries, Bill Gates, aliens or Bill Gates the alien, is irrelevant to the minor pissants that we are. It’s happening. It’s happening and we can’t do a damn thing about it. At least I know that I can’t do anything about it.

I’m not going to tell anyone that they should just give up, give in or stop caring.  What we need to do though is take advice from the likes of David Goggins who teaches us how to “Embrace the Suck”.

“Embrace the Suck” is a term that Goggins takes from Military Slang, meaning to consciously accept or appreciate something that is extremely unpleasant but unavoidable for forward progress.

The Suck that we need to embrace is probably the hardest thing we’ll have to do in our lifetime. I can think back to any number of unpleasant things that I’ve had to deal with and I know that you can too. A death in the family. A major accident. Bad news on your health. Having been forced to listen to Mariah Carey. Losing a job. Losing a business. There are things that we’ve been through that even I can’t easily write about; it’s just too painful. What makes this variety of suck so bad is not having a viable outlet to deal with it. Most people in Panama can’t visit and sit with friends, can’t go to a bar to drown our sorrows, can’t indulge in our favorite outdoor activities. We’re stuck in one spot with our own particular suck and after 4 months I’m sure we’ve seen everything that we care to see on Netflix.

So now it’s time to Embrace the Suck. It’s time to just say, “You know what, screw this, I’m not going to let this amount of suck bother me anymore!” The sooner we come to the realization that this isn’t going away soon, we can stop looking at the now and start looking towards the future. First, let’s identify our problem.

  1. Pandemic – could get sick or worse – check!
  2. Can’t leave the house – check!
  3. Shattered economy – check!

I’m sure we could create some sub-categories, but regardless of however you define your perspective on The Suck, it all reverts back to “I have no control over this!”

So, let’s focus on what we can control. We can’t control the now, but we can control a not-so-distant future. If you believe that this Pandemic only exists because the USA is in its election year, then your new future starts in just 4 months. If you believe that the vaccine coming out of Oxford, Monsanto or Pfiser is going to save us all, then you have about 8 months depending on how quickly they care to distribute it to the 3rd world countries. If you don’t believe any of that “hype”, then it’s a crap shoot. Me, personally, I’m guessing that my businesses are going to start to take off again around January 2021. Maybe I’m overly optimistic, maybe I have secret alien intel or maybe I’m just Embracing the Suck.

Whichever your focus, you need to get moving on your plan and put it in to action immediately.  One way or another, this will get better and it’s up to you whether you stay down here at rock bottom, or prepare to follow the rest of us towards something amazing.

Here are a few things you can do to prepare and therefore Control to some degree, this suck that sucks so much:

  1. Get in shape. If you’ve done nothing but Netflix marathons since April, your muscles are about to atrophy, somewhat like the singing talent of Mariah Carey. There are thousands of free workout routines that you can find on the internet. Sure, “everyone” is joking about how they’re getting fat, but it ain’t going to be funny in another 4 months when your family can’t tell where the couch starts and your ass begins.
  2. E-Commerce – e-commerce is anything but new, but what IS new is the common denominator that we all live; being stuck at home. We have a system now for home delivery for just about anything. In Panama, E-Commerce is recognized as one of the authorized businesses to be operating. If you have/had a business in Panama and haven’t gotten a home-delivery service set up yet, you’re behind the curve (unlike Panama, who hasn’t even been able to flatten the curve yet). Take a week to come up with a business idea and DO IT!
  3. Practice Gratefulness. What does that mean? Well, if you’re reading this, you’re alive. Be grateful for that and live a little. Educate yourself more, if you’re so inclined get closer to God, learn something new about your children – take time to ask them what they like to do or what they would like to be.
  4. Learn how to make the perfect cup of coffee. Anyone can make coffee. If you’re Queen Sayira, even she’ll tell you that “burnt coffee is still coffee… …now drink it!”. Did you know that you can get a different flavor out of the same coffee bean depending on whether you soaked it before making it? The difference in temperature is also extremely important. Look it up on google – it’s not a joke. Especially the burnt coffee part… she wasn’t joking then either.
  5. Train yourself on a new, more efficient and beneficial routine. Get up early. Plan your day. Eat on schedule. Do your workouts on schedule. The mind can be tricked into working better in your favor.
  6. Stop using the internet so darn much. Actually, you don’t need anything more than my reports. So, get up, make the perfect coffee, read my report and then stay away from the internet for the rest of the day.
  7. Incorporate laughter and humor in your day. I’ll leave that one up to you. My source of humor comes from joking around with my kids. You know what makes you happy. Just Embrace the Suck and obey the Brettster.

Yoli’s Summary

Children under the age of five with mild or moderate symptoms of COVID-19 could transmit the virus “as much as other age groups,” according to a study published Thursday by the Jama magazine of the American Medical Association.

The study opens the debate on the safety of reopening schools and kindergartens.

The team of scientists saw that “children under the age of five with COVID-19 have a higher viral load than older children and adults,” explained lead researcher Taylor Heald-Sargent.

“This may suggest increased transmission, as we see with respiratory syncytial virus,” Heald-Sargent explained to the American medical publication.

In addition, I consider that “this has important public health implications, especially during discussions about the safety of reopening schools and kindergartens.”

The study “was not designed to prove that younger children spread COVID-19 as much as adults, but it is a possibility,” added Heald-Sargent.

This coronavirus researcher works at the Ann & Robert H. Lurie Children’s Hospital in the city of Chicago in the United States.

However, he considered that it must be taken into account “in efforts to reduce transmission while we continue to learn more about this virus.”

The team analyzed 145 patients with mild to moderate COVID-19 in the first week of symptom onset and compared viral load in three age groups, children younger than 5 years, children ages 5 to 17, and adults ages 18 to 65. years.

Children younger than 5 years with mild to moderate COVID-19 have “much higher levels of genetic material for the virus in the nose compared to older children and adults,” the study added.

CORTIZO ORDERS TO INCORPORATE PRIVATE SECTOR MEDICAL STAFF TO PUBLIC HOSPITALS TO COMBAT COVID-19

Given the demand for medical personnel in public hospital centers due to the COVID-19 pandemic, the President of the Republic, Laurentino Cortizo, instructed the health authorities to incorporate medical personnel from the private sector to public hospitals.

In his Twitter account, the president reported that he also instructed the directive of the Ministry of Health (Minsa), to accelerate actions aimed at caring for personnel fighting the pandemic, this in view of the visible exhaustion of active personnel and contagions and COVID deaths. that have been denounced by the health unions.

Likewise, Cortizo said that, if the hiring of foreign doctors is required, this process will be subject to compliance with the country’s legal regulations.

Currently, only doctors and health professionals can work in the country with their suitability duly issued by the Technical Health Council. In statements to TVN News, Julio Osorio, president of the National Negotiating Medical Commission, expressed that the medical unions are not opposed to the bringing to Panama of foreign professionals, as long as they comply with the requirements established in the Sanitary Code, the laws and everything that is needed to practice in the country.

He recalled that the Health Code foresees that in a national emergency situation the Government can make this type of call, but explained that the problem lies in two points: on the one hand, the foreign doctors that are hired have to be of the necessary and required quality according to the Panamanian academic curriculum.

And, on the other hand, that it cannot be a “political measure”. He recalled when two governments ago brought in a group of foreign doctors who supposedly “caused mourning for families in the province of Coclé by causing the death of babies and even a mother.”

Facts that justice has not clarified what happened, according to Osorio.

CORTIZO IN TAKING POSSESSION OF APEDE: PANAMA WILL BE ONE OF THE FIRST COUNTRIES TO RECOVER AFTER THE PANDEMIC

Cortizo gave a virtual speech during the inauguration of the new Apede board of directors.

The President of the Republic, Laurentino Cortizo, said that the estimates of international organizations indicate that Panama will be one of the first countries to recover economically after the Covid-19 pandemic. However, this can only be achieved if all sectors work together.

This was expressed by the Panamanian president this Thursday, July 30 at the inauguration of the new board of the Panamanian Association of Company Executives (Apede).

The president indicated that the projections of the Ministry of Economy and Finance point to a drop of 9% during 2020, with a recovery of 4% by 2021. This calculation is in line with what was expressed by Bank of America analysts who expect a decline. of gross domestic product of 10% this year and an improvement of 5% by 2021.

Despite this scenario, Cortizo said that Panama will recover “earlier” than other more developed countries. He emphasized that by 2021 and 2022 the Panamanian economy will grow and “recover”.

The President, on the other hand, reiterated that the Government has established a process of reopening the economy by blocks or specific activities, which will take into account hospital capacity and risk levels.

Cortizo pointed out that a working group was set up a week ago to analyze economic activities to determine which ones can open based on health indicators.

“We have a long way to go, while we do not have the vaccine we have to know how to live with the virus,” he said.

Cortizo explained that his plan is considering the five Panama, made up of all the socioeconomic sectors of the country.

The president stated that initiatives are also being taken into account to maintain the greatest number of jobs and in search of a worker-employer balance. In addition, work is underway to decrease the number of procedures to streamline investment in the country.

He recalled that he has already submitted six bills to the National Assembly to streamline the country’s economy, such as the tax relief bill, the “Pronto Pago” which contemplates a 10% reduction to taxpayers who pay their taxes on time.

Likewise, he indicated that they have already disbursed 2 million dollars as part of the financial plan that the Government will give to the country’s productive sectors. He announced that as of August 3 the Savings Bank and the National Bank of Panama will be receiving requests for financing to access the funds (150 million dollars) that will go to small and medium-sized companies, as well as to the sector agricultural.

For her part, the new president of Apede, Elisa Suárez de Gómez, assured in her inauguration that her union is declared in permanent session to offer sustainable and viable contributions that allow the revival of the Panamanian economy.

Suárez de Gómez emphasized that this group, starting this Thursday, July 30, will offer a more dynamic rhythm than the one it has always contributed, due to the different times and challenges they face.

“The country is at a decisive moment fighting a virus that attacks our weaknesses, and to continue we must recognize our mistakes. Apede will remain free of vested interests, we will know how to be allies of those who make the decisions, but we will not be complicit, ”explained Suárez de Gómez, who replaces Mercedes Eleta de Brenes, under the command of Apede.

“We are willing to go the extra mile … we will never give up because it is not an option,” he said.

PROJECT OF LAW 354 ON EMPLOYMENT PROTECTION IN COMPANIES AFFECTED BY COVID-19 GOES TO THIRD DEBATE

The plenary session of the National Assembly (AN), approved in second debate the draft Law No. 354, which establishes temporary measures to protect employment in companies affected by COVID-19, and dictates other provisions.

The project is temporary, parallel to the Labor Code and seeks to guarantee the protection of the 276 thousand workers who are in a suspended contract, by establishing an extension until December 31, 2020.

During the second round of discussion of the initiative, modifications were made to articles 2, 3, 5,9,10 and 13; and 6 was removed from the original document.

Víctor Castillo, president of the Work Commission pointed out that there are important points such as the protection “of the maternal jurisdiction and the bond according to the reality of the country”.

Castillo expressed that, to the project presented by the head of Labor and Labor development, Doris Zapata, 80% changes were made during the first debate in “the benefit of workers and that they have how to defend their jobs that are a priority in that company and be given the right opportunity to receive their money at any given time”.

The law would only apply to about 20 thousand companies that have reported suspension of contracts or reduction of working hours, while the other 70 thousand companies, registered with the Ministry of Commerce and Industries, which have not reported any labor damages, will remain in the full application of the current Law.

VACCINES AGAINST COVID-19 MAY BE AVAILABLE TO PANAMA IN APRIL OR JUNE 2021

The vaccines are still in the trial phase.

Dr. Francisco Sánchez Cárdenas, a member of the advisory council, reflected on the behavior that the population should have when it is possible to control the coronavirus pandemic in Panama.

In Cárdenas’s opinion, citizens should not be trusted and recalled that biosafety protocols should be maintained.

“I ask the population that when the control of the pandemic begins that we do not take it as a triumph. The triumph will only come when the vaccine is ready. While: masks, soap and water and distancing,” said the neurosurgeon.

Regarding the vaccine, the doctor considers that it will probably be available for Panama in April or June 2021.

According to the specialist, controlling the infection curve would take the entire month of August, because there is still a lot of work to do.

“The contagion figures will not go down. This is not a common cold. It will take the month of August to control the infection curve. Many people will die, unfortunately, still,” the doctor warned days ago.

Following this line, he added that the effects of the traceability strategy, new treatment protocol, the home medication kit and staff reinforcement will be seen in August and September.

On the other hand, the health professional is of the opinion that the fight against COVID-19 was not in hospitals, but in communities.

“Perhaps the world is losing the battle, but not the war. Perhaps the battle was not in hospitals, but in the communities where it is being waged now,” he said.

PANAMA PREPARES TO ATTRACT VOLUNTEERS IN THE SEARCH OF A VACCINE AGAINST COVID-19

Panama has been conducting clinical vaccine studies for more than 10 years; however, this pandemic has marked a new challenge based on the behavior of the virus in our population.

Given this reality, an agreement was signed that consolidates the formation of the Vaccine Research Consortium COVID-19 Panama, between the Institute of Scientific Research and High Technology Services (INDICASAT-AIP) and the CEVAXIN Research Center, with the Guarantee of the National Secretary for Science, Technology and Innovation (Senacyt).

This consortium is made up of a group of more than 10 health professionals, including scientific researchers, epidemiologists, nurses, laboratory personnel, pharmacists, and an advisory group with experience in research studies, to ensure that Panama has full access to the vaccine.

The national secretary of the Senacyt, Víctor Sánchez Urrutia, pointed out that “there are few populations like ours that are being evaluated, and it is important that the quality of the vaccine be evaluated for all types of ethnic groups, because this way we are sure that when the vaccine it is believed to be efficient and effective for us as well. “

“The idea of creating the Consortium is to maximize our chance of being able to earn the chance to participate in these clinical trials,” said Urrutia.

Selection of volunteers

Before being able to start considering volunteers for the study of vaccines in Panama, approval must be obtained from the Bioethics Committee, then the participant must sign an informed consent document and will have a rigorous medical control. “Not all patients are going to be able to participate. It depends on a series of criteria that the doctor evaluates in order to know if that patient meets the criteria to be included in the study,” Dr. Digna Wong, coordinator, explained to TVN Noticias. General of the Center for Scientific Research and Transnational Medicine of INDICASAT-AIP. Our country has already had experience in other important vaccine research studies such as Pneumococcus in which some 7,000 children participated, Human Papilloma and Hepatitis A.

We are now in a race against time and based on that, studies are being worked on simultaneously to be able to extract information that is effective to be able to say if a vaccine is effective and safe. I see it as optimistic, we hope that by December we can have vaccines that are approved for use in at-risk populations, by this I mean health workers, perhaps law enforcement personnel, also those over 65 years and with chronic diseases. And maybe by the middle of next year or the end we will have a licensed vaccine, “concluded Dr. Wong.

Research studies will be supported by a policy in case of side effects in clinical trial participants.

PANAMA IS READY TO BE THE COVID-19 VACCINE DISTRIBUTION CENTER FOR LATIN AMERICA

Panama, through the Regional Logistics Center for Humanitarian Assistance (CLRAH), is preparing to distribute the COVID-19 vaccine to all of Latin America, once it is approved by international organizations.

This was confirmed by Alberto Sierra, executive director of this dependency of the Ministry of Government (Mingob), who reported that these facilities have 1,200 square meters of cold room, intended for the conservation of medicines and once the CLRAH vaccine is available. It will be able to become a distribution center in the same way that humanitarian aid has been brought to some 30 nations on the continent in recent months.

“The plan is ready and we are just waiting for a vaccine to be ready and approved by international organizations to support its distribution,” said Sierra.

The Center together with the Department of Pharmacies and Drugs of the Ministry of Health (Minsa) have reviewed all the necessary protocols and the people trained to handle this very special movement.

COVID-19 LEAVES SCARS ON THE LUNGS OF CERTAIN PATIENTS

Pulmonologists agree that Covid-19 is leaving scars in the lungs of patients who were severe and outgrowed the virus and in those who required mechanical ventilation.

The specialist Fernando Márquez stressed that this is known as “pulmonary fibrosis”, which causes, at times, the person to agitate.

In addition, he explained that they are observing cases of “pulmonary hypertension” and “organized pneumonia.”

Covid-19 leaves sequelae in seriously ill patients

The medical community agrees that it is too early to draw conclusions about the future effects of Covid-19 in infected patients, especially those who were in the most serious condition, hospitalized both in wards and in intensive care units.

However, because it is a respiratory disease, there are specialists, such as pulmonologists, who are observing and analyzing some sequelae of the virus in those patients who required to be connected to mechanical ventilators.

One of them is Fernando Márquez, who stressed that between 30% and 40% of patients with severe Covid-19 who needed artificial respiration may be left with sequelae.

To get an idea, the estimates of the Health authorities indicate that 85% of those who tested positive for the virus in the country will not need to go to a hospital, 10% will go to the ward and 5% will go to intensive care. This last group is the one that generally needs mechanical fans.

In Márquez’s words, they have been observing three types of complications in seriously ill patients, once they are discharged. “Pulmonary fibrosis”, which are scars in the lungs that cause, at times, the person to agitate.

The second is “pulmonary hypertension,” which causes shortness of breath and causes the heart to overload.

And the third is “organized pneumonia,” in which an area of the lung remains inflamed for a long time, even though the person no longer has the virus.

Generally, these patients will require oxygen and other pulmonary rehabilitation therapy, both in the short and long term.

“The sequelae vary, but some patients will remain oxygen-dependent at home and others will have limitations in lung function,” he said.

Lack of pulmonologists

One of the great challenges for the Health system once the pandemic is controlled will be to provide adequate care to these patients, since there are few pulmonologists in the country.

Patients in wards or intensive care with Covid-19 may have future complications in the lungs.

According to Márquez, in Panama there are approximately 30 active pulmonologists and, of those, the majority is dedicated to caring for patients with Covid-19 for about 16 hours per day in hospitals.

“That means that at the moment it will be very difficult to follow up with an outpatient consultation for these seriously ill patients who have overcome the virus. And in the case that we control the pandemic, we pulmonologists will be the last group of specialists that will open the outpatient clinic, due to the nature of the disease and our specialty,” he said.

The pulmonologist noted that all those patients who left intensive care will have to undergo pulmonary function tests to determine which ones will require oxygen or rehabilitation.

“This will be a topic to talk about for many years, because there will be a large number of people with sequelae,” said the specialist, who noted that with the increase in cases in recent months, they went from seeing five hospitalized patients per day to 20.

Also a pulmonologist Reynaldo Chandler said that people who have faced the most severe form of the disease, when they recover, develop limitations in their functional capacity, which will prevent them from recovering after several weeks.

He specified that these patients can present changes in the structure of the lung and also in its function.

“What is being investigated is whether these patients may have a scar in the lung tissue in the long term. Generally, the person who survives severe lung disease develops scars that make the tissue quite stiff. That forces the person to make a greater effort and gives the sensation of shortness of breath, “Chandler explained.

According to the specialist, people with these scars on the lungs can lead a normal life, but with some degree of limitation regarding their ability to carry out physical activities. However, he said that more scientific studies related to the subject are needed.

Oximeters

One of the main recommendations of pulmonologists to health authorities, to avoid complications to the health of positive patients is to measure their oxygen levels.

This is one of the reasons why the Ministry of Health and the Social Security Fund have now chosen to deliver oximeters (oxygen meters) to patients with mild Covid-19 who are in isolation at home or in hotel-hospitals.

According to the doctors, one of the characteristics of patients with Covid-19 is that they often give the impression of being well, but when they are evaluated they have low levels of oxygen in their body, which can cause death. In these cases, timing is key.

MINSA OBSERVES GOOD RESPONSE TO TRACEABILITY AND HOPES TO FLATTEN THE CURVE IN EASTERN PANAMA

The Deputy Minister of Health, Ivette Berrío, announced on Thursday that the Panama East region has had a good response to the traceability strategy implemented, so they hope that this will be reflected in the flattening of the curve in this sector.

“We have seen that the numbers are already giving the impression that they are beginning to drop, especially the new cases today are less than two days ago, the contacts are also lower, the figures are responding to the intense work being done by the traceability teams of the area. Panama East has a good response and we hope that this response will soon be observed on the curve that indicates to us that the entire traceability strategy is paying off, ”said Berrio.

She added that civic clubs, the living forces of the community and others who want to be part of the initiative to contain the virus in this region have joined in this strategy.

The Regional Director of Health of East Panama, Carlos Batista, described the traceability strategies as successful since it allows to detect who the infected are, their contacts, locate them in their homes, isolate them, monitor them through the unified traceability teams, monitor and supply them. everything you need.

He explained that the Panama East region is comprised of the township of San Martín, Chepo district, Cerro Azul, part of Pacora and the Chimán and Balboa districts, which present a cumulative total of 2,350 cases, of which 297 remain as active cases.

The statements were given during a meeting held this Thursday at the Community Traceability Operations and Control Center (COCYTC) in East Panama located in the Chepo district, which aimed to continue monitoring actions that seek to reduce infections by COVID-19.

END NOTES

I know that many of my readers can relate to this.  To become old and wise, you must first have been young and stupid.  If you’ve retired to Panama, it’s highly likely that you’re a part of this coveted category.  There are some out there reading who are also thinking to themselves, “I can’t wait to be old and wise”.  And all I can say to them is, just take it slow.  Live life right and earn your spot.  There are yet those other youngins who are thinking, “You’re wrong Brett, because I’m very wise.”, and they would be wrong… very very wrong.  You see, you can be young and stupid, you can be old and wise, but you can’t be young and wise. That’s… nope, that’s just not possible.

But the worst thing that could happen to you is to fall in to the last category; the category of “old and stupid”.  That’s the worst group to be in.  Don’t ever let yourself get in to that category.  I did it and now I’m trying to play “catch up”, to not be stupid before I get any older.  Listen, youngin, you can’t avoid getting old, but you can avoid being both old and stupid.

Stay safe… Stay healthy… And by all means (unless you’re out gaining wisdom) STAY HOME!!…
