Horrors and Thankfulness; Covid-19 vaccine; Plan to increase ICU rooms; Tests of effectiveness of therapies against Covid-19; One Last Thankful Thought

What.  A.  Week.  Oh hell, what a MONTH!  I can’t even begin to fathom the possibility that I could be the only person that thinks that the month of November in Panama was completely UNFAIR!  And “unfair” isn’t even the right word.  We got skimmed by two hurricanes which flooded major communities and eventually led to the declaration of Bambito in Chiriqui being declared “uninhabitable”.  That’s right people, time to move out so we can demolish your homes so you never come back!  Holy shit, that really puts life into a certain perspective that I never imagined I’d have to consider.

A friend and client recently asked me if I was a Navy Seal in my military career.  I can see how he got it mixed up seeing as the Navy Seals are an amazing and respectable force to be reckoned with… …and then there’s me.  Hahaha.  I corrected him by saying that I was a lowly Intelligence specialist; I was a US Army Interrogator.  There was a pause and I added, “I used to play Good Cop, more often than not.”.  The truth is, during peacetime, there isn’t anyone to interrogate, so they send us off to do all kinds of Intelligence-ish types of things.  After that I worked for Special Operations.  Some things were quite horrible.  Most of those horrible things were done by nature.

During Hurricane Mitch in 1999, I witnessed the devastation that this absolute piece of shit hurricane did to the country of Honduras.  This thing hit the country with the power of a CAT 5 storm, receded to the ocean and then said, “nope, I ain’t done with you yet”, crashing once again into Honduras as a CAT 5.  Not too many countries in the world can claim to be devastated twice by the exact same hurricane.  We were assigned to the coastline as a relief team to help police up the beach for bodies. Basically the search and rescue quickly transformed into search and recovery.  The task was essentially just all of us waking up each morning, going to the beach and picking up more bodies that had washed up from the tides the evening before.  I don’t remember how many days we were there.  Maybe it was just a week, but it seemed like forever.  It seemed like a never-ending mission.  Even the day I left, the team that stayed were still picking up bodies.  A memory that will never leave my thoughts was also a point of my curiosity.  Everybody we found was face down with the head facing land and feet towards the ocean.  It was as if the ocean itself would return them to dry land, placing them in the most comfortable position possible, regretting having taken them in the first place.

And believe it or not, there’s been worse.  But whether it was the devastation of Mitch, a plague in Ecuador, civil unrest in Guatemala, the onslaught of floods in Chiriqui, or the ever-increasing list of new cases of COVID, we must move forward.  Manuel Antonio Noriega once said, “Ni un paso atras”, “Not one step back.”.  Although a military unit has the capacity to retreat and regroup, the flow of time does not allow us to do anything but move forward.  As horrible as we think we may have it now, we must, as it is in our nature, move forward or we’re doomed to get left behind emotionally, financially and morally.

So what’s my point, other than babbling on during what could only be a comatose diatribe induced by my overindulgence of turkey and pumkin pie?  The more horrors we see only gives way to more reasons to be thankful for what we have and the life we were gifted with.  Whether you believe in God or not, being thankful is being grateful.  Perhaps it’s just being grateful to the universe, but acknowledging the fact that life could suck considerably more than it does right now.

With that being said, I’m thankful for God in my life, my health, my family and their good health, my continued employment, the good people that have aligned themselves with my path, the opportunity to help others, and the continued wisdom that I’ve obtained through so many fortunate instances and the good people who help to guide me.  It hasn’t always been a bed of roses, nor will it always be a bed of roses, but whatever it is that life throws at me, I know that we’ll make it through given that which I’ve already experienced.

One in five Covid-19 patients develops a mental illness, symptoms appear during the three months after testing positive for coronavirus.

A new medium-term effect appears among Covid-19 patients. According to a study published in The Lancet Psychiatry, people who have suffered from coronavirus are more likely to develop a mental illness within three months of testing positive.

In this 90-day period, 20% of the patients studied — one in five — manifested anxiety, depression or insomnia for the first time.

To conduct this study, researchers at the University of Oxford in the United Kingdom analyzed the medical records of 69 million people in the United States, of whom more than 62,000 had had coronavirus. Their conclusions, they say, could easily be extrapolated to the rest of the world.

“People are concerned that Covid-19 survivors are at higher risk of mental health problems and our findings show that this is very likely,” Dr. Paul Harrison, professor of psychiatry at the University of California, told Reuters. Oxford

Although anxiety, depression and insomnia are the most common illnesses among recovered patients, the researchers also found significantly higher risks for dementia, a brain-impairing condition.

“Anxiety, depression and insomnia are the most common illnesses among recovered Covid-19 patients”

“This is likely due to a combination of the psychological stressors associated with this pandemic and the physical effects of the disease,” explained Michael Bloomfield, a consultant psychiatrist at University College London.

As psychiatrist Margaret Seide assured the medical journal Health, it is common for insomnia, anxiety and depression to increase after overcoming a trauma. “This is precisely what is observed in the period after passing Covid-19.”

“Facing the possibility of not surviving a situation is terrifying,” Seide added. “Most of us have the luxury of not having to think often about our death. It makes sense for such a situation to have effects on mental health, particularly for those who had a difficult course of illness that included hospitalization or respiratory distress. “

“It makes sense that such a situation has effects on mental health, particularly for those who had a difficult course of the disease”

The conclusion of the study is different for people with pre-existing mental illness, as they are 65% more likely to be diagnosed with Covid-19 than those who do not.

Simon Wessely, a professor of psychiatry at King’s College London, told Reuters that this finding coincides with similar ones in previous outbreaks of infectious diseases. As he explained, “the coronavirus affects the central nervous system and, therefore, could directly increase subsequent disorders. But this research confirms that this is not the whole story and that this risk is increased by previous health problems. “

WHAT WILL HAPPEN TO SUSPENDED CONTRACTS AND WAGES CUT AFTER DECEMBER 31?

Making a bit of history, on August 3 the administration of President Laurentino Cortizo passed Law 157 that establishes temporary employment protection measures. This regulation has a birthday date to lose effect and is December 31, 2020.

Article 1 clearly establishes that the law “will apply exclusively to companies that closed, totally or partially, their operations, from the beginning of the national state of emergency until December 31, and to workers whose contracts have been suspended by authorization. tacit or express of the Ministry of Labor and Labor Development, in this period ”.

According to Jean Pierre Leignadier, president of the Chamber of Commerce, conversations are currently being held with the Minister of Labor and Labor Development (Mitradel), Doris Zapata, to determine the future of jobs in Panama.

Despite the reopening, many hard hit companies have not been able to open and others have done so due to fear of a new closure and with their limited workforce.

“There is a low percentage of activation of suspended contracts and we are talking about what options or alternatives we can implement other than December 30,” said Leignadier. From the Mitradel they confirmed the conversations with the businessmen.

“At the moment we are evaluating the options and before making any decision, we will consult with the sectors involved,” said Cristal Lawson, advisor to the Superior Office.

The President of the Chamber of Commerce affirmed that in the hands of the minister there are several scenarios to take into account and that they want to recover as many jobs in the shortest possible time.

“The Ministry of Labor is also looking at alternatives, because in the end what we all want is to see how we recover jobs as quickly as possible, but there is a reality that the country is facing and that reality cannot be ignored,” said Leignadier.

The Executive in case of extending the measure, should return to the National Assembly to discuss the extension of Law 157.

The legislature is currently in recess, so President Cortizo should call extraordinary sessions.

GOVERNMENT ANNOUNCES AGREEMENT WITH PFIZER TO ACQUIRE 3 MILLION DOSES OF VACCINE FOR COVID-19

Brett’s Comments

3 million doses means that 1.5 million people will potentially get the vaccine.  The Pfizer vaccine requires 2 doses.  This represents roughly 33% of the Panamanian population.  Panama will still have to acquire another 3 million doses to make it potentially effective.

The lab reported that the trials show that their vaccine is 95% effective.

The Panamanian government reported this Wednesday, November 25, that it reached an agreement with the pharmaceutical company Pfizer Panama and BioNTech SE to acquire 3 million doses of its vaccine against Covid-19.

According to the announcement made by the Presidency of the Republic, the terms of the agreement are based on the timeline for deliveries and the volume of doses progressively during 2021. It is specified that this will be given prior approval of the Ministry of Health (Minsa ).

“The objective is to ensure that it is a duly authorized and licensed vaccine, to permanently control the current coronavirus pandemic and make it available in Panama in the shortest possible time,” said the President of the Republic, Laurentino Cortizo, according to a statement.

Pfizer reported last week that trials show that its vaccine is 95% effective.

Likewise, it is emphasized that the first condition for this acquisition is that it be a vaccine duly tested and authorized by the World Health Organization (WHO).

For now, there was no information on the cost of buying this batch of vaccines.

On Wednesday, November 18, it was reported that the Cabinet Council approved the purchase of one million 92 thousand doses of the vaccine for Covid-19 from the British pharmaceutical company AstraZeneca UK Limited, in association with Oxford University, for an amount of $ 4 million 368 thousand.

This laboratory reported that the vaccine is in phase III and according to the latest report it has an average efficacy of 70%.

It was previously reported that the Government had already approved about 48 million dollars for the purchase of vaccines against this virus.

CSS PRESENTS PLAN TO INCREASE CAPACITY OF THE INTENSIVE CARE UNIT

An intermediate care area is being considered that allows that when patients leave the Intensive Care Units they can be evaluated and given the best follow-up before being discharged.

An intermediate care area is being considered that allows patients to be evaluated when they leave the Intensive Care Units.

On November 24, Enrique Lau, general director of the Social Security Fund (CSS), gave a report on the situation in Intensive Care Units in which it is planned to send patients to hospitals outside of Panama City.

He explained that an intermediate care area is being considered that allows when patients leave the Intensive Care Units they can be evaluated and give them the best follow-up before being discharged.

HOSPITALS

At a press conference, it was indicated that a strategy is being made so that the Dr. Arnulfo Arias Madrid Hospital Complex continues to be a hospital that mostly attends patients for Covid-19 with commitment.

Dr. Susana Jones Cano Hospital will attend to surgical complexities, catheterization problems and neurosurgical situations.

The Irma de Lourdes Tzanetatos Hospital will continue to treat non-covid conditions that correspond to the second and third level of complexity, such as neonatology.

A hospital is being activated in the district of December 24 that has 43 beds where the Wound Clinics, Day Hospital among other conditions will be attended.

“We are in the process of installing a field hospital donated by the United States Embassy that will be located in # Panama Oeste near the La Chorrera hospital.

“In coordination with the Ministry of Health, the San Miguel Arcángel Hospital is providing us with 30 beds to meet the needs of patients who are in the surgery phase that need to be attended immediately.”

“We are working on the City of Health to make 84 beds available to the population for Intensive Care Units, 38 beds for Respiratory Care Units, and 154 beds for patients with moderate conditions.”

PERSONAL

“We are appointing 30 general practitioners to reinforce and guarantee better service to the population. They will be in Special Respiratory Care Units, and in Intensive Care Units ”, revealed Lau.

He assured that the doctors and the health team of the first line of intervention maintain the commitment to serve and give the best possible care to the population.

“The large volume of drug consumption at the international level has made Panama not escape from the reality of having a shortage of some medical supplies,” he said.

Lau reiterated that, if people keep the biosecurity measures that have been given so far, especially young people who are the ones who are carrying the virus to their older adults, there is no need for the number of cases to increase.

INCREASE RT TO 1.17; ABOUT 20 PATIENTS ARRIVE AT FIGALI

Brett’s Comments:

I have a feeling that this is just one more step towards an excuse to lock Panama down again around mid-December.  If we lock down again, the closure of businesses will be exponential.

The Ministry of Health reported yesterday that the effective reproduction number (Rt) for the epidemiological week that ended last Saturday is 1.17, which represents an increase compared to the previous week when it was 1.11.

Since November 21, the Health authorities began to send patients to the Figali Convention Center. So far there are about 20 there.

ADULTS, MAIN DRIVE OF A SECOND WAVE OF CORONAVIRUS CASES

Health authorities expect a rebound in cases.

In recent months, with the return to routine and commercial activities, in most countries it has been seen that adults between 20 and 40 years old may be driving a “second wave” of Covid-19 cases.

In a recent study by researchers from Emory University, Atlanta, USA, and the Georgia Department of Public Health, it was found that people under the age of 60 tend to be the main drivers of super spread events, such as the Covid-19.

This, fundamentally, because they are the ones who leave the house the most and are responsible for a large part of the transmission of the new coronavirus and the origin of the outbreaks.

According to research, people in that age group are three times more likely to infect others compared to those over 60.

Faced with this, the Pan American Health Organization (PAHO) was emphatic and invited the population to consider that today social discipline is more necessary than ever, because the ghost of a second wave of the epidemic is latent and could also occur in Panama.

“Now that families are resuming the activities that they had interrupted when the pandemic began, and that December holidays such as Mother’s Day and the end of the year are approaching, PAHO emphasizes the importance of all of us redoubling care measures personal and protect especially those who are most vulnerable ”, stated the international organization.

For Public Health specialist Jorge Luis Prosperi, this increase in cases or second wave is not an alarmist assertion, as some people who are not convinced of the seriousness of the epidemic still affirm. “It is a reality and we have to face it with great individual and collective responsibility,” he said.

LA CHORRERA AUTHORITIES ANALYZE THE IMPLEMENTATION OF MORE SEVERE MEASURES TO AVOID COVID OUTCOMES

The authorities of the La Chorrera district met this Tuesday, November 24 to analyze the possibility of taking more energetic measures to avoid the spread of COVID-19, which in recent days has been increasing. And it is that, people insist on non-compliance with curfews, as well as the rest of the decrees that have been established by the National Government to stop the spread of the virus. Cecilia Campos, representative of the Chamber of Commerce, indicated that the closure is not an option for this union, since it has been two months of great effort and those businesses that are open today have done it with a lot of sacrifice. Next Friday they will meet again, but this time they hope to have the participation of the mayors of the five districts that make up the province of Panama Oeste, to analyze what measures will be taken in this regard.

DIDI CREATES GLOBAL FUND OF $ 10 MILLION TO SUPPORT DRIVING PARTNERS WITH COVID-19 AND ENABLES IT IN PANAMA

The DiDi mobility platform enabled in Panama its economic aid plan aimed at driving partners who are diagnosed with covid-19.

The mobility company DiDi reported that it created a global fund of $ 10 million to provide financial support to its driving partners in case they are affected by the covid-19 virus, as part of its Corporate Social Responsibility program.

The mobility platform announced that the economic benefit plan for drivers who are diagnosed with the virus, was enabled in Panama, in accordance with terms and conditions

Its objective is, firstly, prevention through isolation and, secondly, mitigating the impact on family income caused by the obligation to respect the isolation period required by the health authorities.

Likewise, and for the driving partners who voluntarily want to receive them, the platform started donating prevention kits consisting of masks and alcohol gel, so that the driving partners can protect themselves and also the users.

At this stage, a total of 500 kits are being delivered. Drivers who wish to receive it can register through the application, specifically in the “Help” section and then in “DiDi’s Actions against Coronavirus”.

“The well-being of those who use the platform is our priority. We continually look for ways to support them and that is precisely the objective when enabling this economic aid plan in Panama. We understand that staying in quarantine has very important socio-economic effects, “said DiDi CEO for Central America and the Caribbean, Pablo Mondragón.

He also added that “we must not lose sight of prevention, so we are also donating these kits to promote compliance with biosecurity measures.”

HOW TO ACCESS ECONOMIC SUPPORT?

He explained that the moment a driver indicates that he suffers from Covid-19, his account is temporarily suspended, to avoid contagion. Once the diagnosis is corroborated, we proceed to the process to request the aid.

For this, you must enter the “DiDi Conductor” application and in the “Help” section, upload an official medical certificate and other information that is requested to start the case study, he said.

After the information has been reviewed and it complies with the requirements, the driving partner will receive, a single time, the approximate amount of one month of the income generated up to that moment, through DiDi.

MAYOR OF COLÓN TAKES A STEP BACK, SUSPENDING THE CURFEW ESTABLISHED IN THE DISTRICT

The mayor added that they began a series of prophylaxis operations to mitigate the wave of violence that is hitting the province.

The mayor of Colón, Alex Lee called the media this morning to announce the imposition of a curfew for this district in order to stop the increase in violence that has occurred in recent days, however, hours later it issued a statement in which it backed down and suspended the measure.

“We clarify to the residents of the district of Colón that after hearing the clamor of citizens, the business community and transporters working for the economic reactivation, we have decided to suspend the announced curfew,” says part of Mayor Lee’s statement.

In this information, the mayor emphasizes that they remain concerned about the wave of violence that afflicts them and together with the National Police and justices of the peace, they will initiate social, day and night prophylaxis operations to seek to reduce homicides that occur during the day and at night in order to safeguard the life, honor and property of the inhabitants of this district.

“We reiterate the call to coexistence in harmonious tolerance and social peace especially during the time that is coming,” says Lee.

It should be noted that in the last 24 hours in Colón there have been two homicides, the attack of a minor inside the facilities of the Manuel Amador Guerrero Hospital and a shooting on the Panama-Colón highway against a sedan vehicle.

THE COVID-19 VACCINE COULD BEGIN TO BE ADMINISTERED IN THE US ON DECEMBER 12

Slaoui’s schedule means that the first vaccines could begin to be administered more than a month before Biden takes office on January 20.

The first Americans could start receiving the covid-19 vaccine at the earliest on December 12, predicted this Sunday the main adviser of Operation “Warp Speed”, the White House team that tries to accelerate medical solutions to the pandemic.

Advisers to the US Food and Drug Administration (FDA) will meet on December 10 to decide whether to approve the vaccine developed by the pharmaceutical company Pfizer and its German partner BioNTech, which this Friday asked for a Emergency authorization for its preparation in the North American country.

If they give it the go-ahead that same day, the US government is “prepared to begin distributing the vaccines within 24 hours of their approval,” said the adviser to Operation “Warp Speed”, Moncef Slaoui, in an interview with the ABC News network.

“We will have the (first) vaccines (distributed) one day after approval, and we hope that people can begin to be immunized, I would say within 48 hours of approval,” that is, on December 12, Slaoui added. .

If that happens, the expert calculated that the United States could return to the closest thing to a new normal “sometime in May” in 2021, when he estimated that 70% of the population will have received the vaccine and a group immunity.

The first vaccines will be distributed proportionally to the population of each state, and those territories will be able to decide who will have priority to receive it, usually among the high-risk population and medical professionals, Slaoui explained.

The Trump administration has given few details about how it will face the challenge of vaccinating the more than 330 million inhabitants of the United States, and has not shared its plans with the team of President-elect Joe Biden, who has warned that this lack Cooperation can cost lives in the country.

Slaoui’s schedule means that the first vaccines could begin to be administered more than a month before Biden takes office on January 20.

The vaccination effort will also have to address the distrust of many Americans towards the vaccine: Only 58% of citizens are willing to have it administered, according to a poll published this week by Gallup.

Pfizer’s vaccine requires two doses administered about three weeks apart, and has shown up to 95% efficacy in preliminary clinical trials, without major safety concerns.

The United States has another candidate that has also demonstrated the effectiveness of its vaccine, that of the pharmaceutical company Moderna, which could request emergency authorization shortly and that, unlike Pfizer, does owe it to the injection of funds from the United States Government. part of its success in the historically rapid development process of the preparation.

PAHO: THERE IS NO TESTING OF THE EFFECTIVENESS OF THERAPIES

Brett’s Comments:

I almost didn’t add this article to the report, but to allow for a bit of adversity, I left it in.  I have little respect for PAHO.  They don’t seem to be doing their own research, but instead tend to regurgitate some ridiculous political agenda to discredit pharmaceuticals that have been working in the field for many clinics and hospitals in several countries.  First it was with  HYDROXYCHLOROQUINE and now with REMDESIVIR.  I don’t understand how they think this helps society… …actually, I’m pretty sure they don’t care about society.  Their motivation is elsewhere.

In the country, medicines such as remdesivir, tocilizumab and convalescent plasma are supplied to patients in the ward and intensive care.

The treatments and therapies that have been provided to patients with Covid-19 do not show any evidence of their effectiveness against the virus.

The Pan American Health Organization (PAHO) reviewed 55 possible interventions for the management of patients with the virus, and found no major benefits in the people who received them.

For example, in the World Health Organization Solidarity clinical trial, the drug remdesivir had no clinically relevant effect on mortality. Other studies and clinical trials in which medicines and therapies such as hydroxychloroquine, convalescent plasma and tocilizumab, among others, were evaluated, did not show effectiveness in their use either.

For the pulmonologist Reynaldo Chandler, scientific evidence must prevail in the fight against Covid-19. “The Ministry of Health is on the right track by separating in advance the vaccines that show that they are effective,” he said.

TREATMENTS AGAINST COVID-19, WITHOUT RESULTS

The conclusion was categorical: so far, most of the research in the field of Covid-19 has very low methodological quality, which makes its use and application difficult.

This approach is part of the country situation report prepared by the Pan American Health Organization (PAHO), in which it refers to the randomized clinical trials carried out around the world on treatments to counteract the aggressive SARS-CoV-2.

The document states that after more than eight months of a pandemic, there is no sample or evidence of the effectiveness of these treatments. It even goes further and makes the following call: “The need to develop high-quality randomized clinical trials that include patients with Covid-19, in order to develop reliable management strategies, remains pressing. This is essential in making decisions based on evidence ”.

CLINICAL TRIALS

What treatments does PAHO refer to? The international body evaluated 55 possible interventions for the management of patients with Covid-19, although it highlights that more than 200 therapeutic options or their combinations are being investigated in more than 1,700 clinical trials.

However, it was found, for example, that the body of evidence on steroids includes 10 controlled clinical studies (RCTs), and shows that low to moderate doses “probably” reduce mortality in patients with severe infection by steroids. Covid-19.

In the World Health Organization (WHO) Solidarity trial, the drug Remdesivir had no clinically relevant effect on overall mortality, the need for invasive mechanical ventilation, or length of hospital stay.

According to PAHO, by combining these results with three other clinical studies, Remdesivir could reduce mortality, the requirements for invasive mechanical ventilation, and improve the time to resolution of symptoms. However, the certainty in the evidence “is low” and more information is “necessary” from studies with an adequate design to confirm its effectiveness.

THE HYDROXYCHLOROQUINE

Regarding the body of evidence on the use of hydroxychloroquine, interferon beta 1-a and lopinavir-ritonavir, including the preliminary results of the trials called Recovery and Solidaridad, there is no evidence of benefits in reducing mortality or in the time for clinical improvement.

Even PAHO specialists indicate that the evidence on hydroxychloroquine suggests that its use probably generates an increase in mortality. In this case, they argue that more information is required from studies with an adequate design to confirm or discard these conclusions.

Another treatment that gained strength during the pandemic was the use of plasma. In this context, the evaluation found that the results of seven clinical trials in which convalescent plasma was used in patients with Covid-19 showed a statistically “non-significant” trend towards a reduction in mortality and the need for invasive mechanical ventilation.

Also in that case, the certainty in the evidence is very low for scientists.

The same occurred in relation to the use of tocilizumab, in which the results of three studies suggest an absence of benefits in mortality with a trend towards a reduction in the requirements of mechanical ventilation and an increase in the speed of resolution of symptoms. Whereas, in relation to ivermectin, colchicine and famotidine, there is very low-certainty evidence, so their effects are uncertain.

In the case of Panama, some of the medications that are supplied to patients with Covid-19 and contained in the “Protect yourself Panama” kit are vitamin C, vitamin D, probiotic, ivermectin, zinc, azithromycin and hydroxychloroquine.

Meanwhile, in more serious patients, remdesivir tends to be prescribed , favipiravir, tocilizumab, and convalescent plasma. Some of these are mentioned in the PAHO report.

VACCINES

Faced with this reality, pulmonologist Reynaldo Chandler -who has advised the Ministry of Health in a pandemic- stated that randomized clinical trials are key in determining the efficacy of a certain drug. “These are not observational studies; they are experimental studies ”, the specialist said.

Chandler said he understands the population looking for a drug to deal with the virus, but stressed that scientific evidence must prevail in this case. “We are still dependent on the production of a vaccine and the Ministry of Health is on the right track to buy the doses in advance,” said the pulmonologist.

Indeed, the Minister of Health, Luis Sucre, reported yesterday that the country is evaluating various strategies to receive vaccine doses on time. To do this, he explained that they will order from the companies Pfizer and AstraZeneca, since neither will have the capacity alone to deliver the doses they are requesting: 4 million vaccines.

In this context, he explained that, although a budget has already been approved to obtain these two vaccines, within the contract it was established that, if any of the companies does not comply, the country is free to acquire them with other suppliers and they have to return the money they were paid.

Currently, the Expanded Immunization Program of the Minsa is working on the projects that are needed to keep these vaccines at the correct temperature.

In turn, this week the Cabinet Council authorized the Ministry of Health to proceed with the purchase of one million 92 thousand doses of the vaccine to prevent Covid-19 from the British pharmaceutical AstraZeneca UK Limited, in association with Oxford University, for an amount of $ 4.3 million.

In addition, on November 18, a resolution was published in the Official Gazette authorizing an understanding agreement with the company Pfizer for $ 48 million, to purchase 4 million vaccines against Covid-19.

While that happens, a rebound in cases is reported in the country. In fact, the PAHO report shows an increase of 37% this month compared to the previous one.

Francisco Sánchez Cárdenas, member of the Health Advisory Council, said that this increase in cases is part of the scenario expected for November and December.

“I understand that an alarm may be raised about the increase, but it is normal because it was within our projections. Now, the important thing is to maintain the adequate number of beds, to carry out the traceability of the cases and for the population to comply with the use of the mask and other biosafety measures ”, the specialist said.

MORE THAN 30 THOUSAND SAMPLES HAVE BEEN TAKEN AT THE EXPRESS SWABS CENTERS

The health authorities revealed that more than 30 thousand samples have been collected in the express swab centers of the Gran Estación, Chepo and the Colegio Remón Cantera; which has become an advantage for Panamanians to know in advance if they suffer from COVID-19.

THE ASTRAZENECA / OXFORD VACCINE AGAINST COVID-19 HAS A 70% EFFECTIVENESS; PANAMA HAS AN AGREEMENT WITH THIS LABORATORY

The laboratory announced that it will present its results very quickly to the authorities for a first approval.

The vaccine against Covid-19 developed by the British laboratory AstraZeneca and the University of Oxford has an average efficacy of 70%, the laboratory reported Monday in a statement.

In some cases, the efficiency is up to 90%, putting it on the same level as those of Pfizer / BioNTech and Moderna, the statement added.

This is demonstrated by the intermediate results of large-scale clinical trials conducted in the United Kingdom and Brazil, the laboratory statement said.

“The efficacy and safety of this vaccine confirm that it will be very effective against Covid-19 and that it will have an immediate impact on this public health emergency,” said AstraZeneca CEO Pascal Soriot.

Specifically, the effectiveness rises to 90% in the case of a first sample of people who received a half dose and then a full dose a month later. Efficacy drops to 62% in another group that received two doses in total one month apart.

The pharmaceutical group did not provide details to explain this difference.

Last week in Panama, the Cabinet Council approved the purchase of one million 92 thousand doses of the vaccine for Covid-19, from the British pharmaceutical company AstraZeneca UK Limited, in association with Oxford University, for an amount of $ 4 million 368 thousand .

With an average of 70%, this vaccine is currently less effective than that of Pfizer / BioNTech or Moderna, which claimed to exceed 90%.

But it has the great advantage of using a more traditional technology than that of its two competitors, which makes it less expensive and easier to store and transport, since it does not need to be kept at a very low temperature.

WAITING FOR APPROVAL

AstraZeneca estimated in the statement that its vaccine is “highly effective” in preventing the disease, and noted that during the tests no participant developed serious forms of coronavirus or had to be hospitalized.

I congratulate “our brilliant scientists” and “all the volunteers in these trials,” said British Prime Minister Boris Johnson, celebrating the “fantastic results”.

However, Johnson recalled that “there is still more security checks to do” before it can be distributed.

The United Kingdom, which has bet very strongly on this project, has reserved 100 million doses.

These preliminary results included trials in more than 2,000 people, of whom 131 contracted the disease.

In total, large-scale phase 3 clinical trials span 60,000 people worldwide and are conducted in the United States, Japan, Russia, South Africa, Kenya, and Latin America.

The British laboratory announced that it will present its results very quickly to the authorities for a first approval.

The efficacy of these vaccines, developed in record time, is raising great hopes around the world, as many countries face a second wave of the coronavirus.

And the restrictions and reconfinements in force are weighing heavily on economies, with an explosion in unemployment and company bankruptcies.

If they quickly get the green light from their respective drug safety agencies, the first Covid-19 vaccines could start being distributed in December in the European Union and the United States.

Thanks to its “simple supply chain,” the Oxford / AstraZeneca vaccine “will be accessible and available worldwide,” Soriot said.

“These results show that we have an effective vaccine that will save many lives,” said Professor Andrew Pollard of the University of Oxford.

The British laboratory claims to be making rapid progress in the planned manufacture of 3 billion doses, which will be available in 2021.

SPUTNIK V IS MORE THAN 95% EFFICIENT AND WILL COST LESS THAN 10 DOLLARS

The FIDR highlighted that the price, less than 10 dollars for a dose, is twice less than the vaccines that use mRMA, that is, the American Moderna and the Pfizer.

According to preliminary data from the second analysis, the efficacy of the Russian vaccine after 28 days is 91.4%

The Russian Sputnik V vaccine showed 42 days after the first dose an efficacy of more than 95% and will cost less than $ 10 per dose, the Gamaleya Center and the Russian Direct Investment Fund (FIDR) reported today in a statement.

According to preliminary data from the second analysis, the efficacy of the Russian vaccine after 28 days is 91.4%.

In this regard, clinical analyzes showed, for an efficacy of 91.4%, a total of 39 infected, including volunteers who received placebo (31) and those who received the vaccine (8).

In total, 14,095 volunteers received the vaccine and 4,699, the placebo, according to the official note.

Currently, 40,000 volunteers participate in phase three, of which more than 22,000 were injected with the first dose and 19,000 received both the first and second.

During the analyzes the Russian experts have not detected “unforeseen” adverse reactions to the vaccine among the volunteers.

The FIDR noted that the price, less than 10 dollars for a dose, is twice less than the vaccines that use mRMA, that is, the American Moderna and the Pfizer.

In addition, Russia has already launched the production of the dehydrated vaccine, based on lyophilization technology, which will significantly facilitate its transport to international markets, including regions with difficult access and others with tropical climates. The first batch of Sputnik V for the foreign market will reach customers in January 2021 on the basis of the agreements already signed with foreign partners.

PROPOSED TO RESTRICT MOBILITY IN DON BOSCO

Brett’s Comments:

Once again, more calls for restrictions.  Step by step and location by location, Panama is moving towards closing down again.  Willie is right though, a total quarantine is NOT feasible.

Given the increase in cases that have occurred in the Don Bosco district, the local authorities believe that it is time to re-establish the measures to restrict mobility in that area.

There are times of pandemic in health, but also economic, which is very worrying, however, a total quarantine “is not feasible,” according to Representative Willie Bermúdez.

In that district, the “parking” and parties are the main cause of contagion and the rebound in cases, where young people are mostly exposed and infect adults.

He has called on the Ministry of Security to be more energetic in controlling such activities that occur very frequently on weekends.

The Don Bosco district had reflected a decrease in active cases, but in recent days there has been an increase.

ELECTORAL COURT ANNOUNCES THAT THE VALIDITY OF THE CARD FOR OVER 70 YEARS WILL BE UNDEFINED

This measure seeks to prevent those over 70 years of age from having to appear at the TE offices.

This morning the Electoral Tribunal (TE) revealed that as of this date the personal identity card of citizens over 70 years of age will be valid indefinitely and the permanent resident card of foreigners will maintain a duration of 10 years.

In addition, the entity pointed out that the cédulas for the rest of the citizens will be valid for 15 years.

“The new validity of the personal identity card of Panamanians over 70 years of age is given so that it can be issued to them indefinitely, thus avoiding having to appear at the offices of the TE,” the Court’s statement said.

The institution adds that those over 70 years of age will only be able to carry out the renewal process if the document is lost, either because they have been victims of theft, robbery, or make any changes to their data that warrant the issuance of a new document.

On the other hand, regarding the resident card, it is reported that it will only be processed in the TE by authorization of the National Migration Service (SNM) for the first time and those that must obtain it due to expiration, deterioration, loss, loss or theft must be redocumented , according to Decree Law 3 of 2008, regulated by Executive Decree 320 of 2008.

The TE emphasizes that those who face the special situations described in Law 25 of 2008, the TE will determine the issuance of the document within the term that is set.

“This follows from Decree 48 of November 24, 2020, which modifies articles 3 and 5 of Decree 1 of February 1, 2017, modified by Decree 21 of May 28, 2020, which bears the signature of the presiding magistrate of the TE, Heriberto Araúz Sánchez; the magistrate first vice president, Eduardo Valdés Escoffery; and the magistrate second vice president, Alfredo Juncá Wendehake “, concludes the statement of the TE.

80% OF DEATHS FROM COVID-19 ARE ASSOCIATED WITH CHRONIC DISEASES

Health personnel attending to a hospitalized positive patient with Covid-19.

In the midst of the Covid-19 pandemic, one of the most vulnerable populations to suffer from this virus are patients with chronic diseases.

According to the Social Security Fund (CSS), the statistics of deaths from coronavirus reveal that 80% of the patients had some comorbidity, associated with chronic non-communicable diseases.

They emphasize that the most common chronic diseases that affect a large part of Panamanians are: blood pressure, diabetes and morbid obesity, as was announced by Dr. Alex González, national executive director of Health Services and Benefits of the CSS.

He said that there are other types of diseases that also produce what is known as an “immunosuppression” in the patient’s state, which lowers his defenses.

“And when COVID-19 makes them sick, the body’s reaction is more acute, and that is why it is important that patients with chronic diseases should take care and control their disease”, emphasized Dr. González.

The director of Health Services and Benefits of the CSS highlighted the importance of having their disease controlled, taking into account the following recommendations:

-In the case of hypertensive patients, they should take their medications daily as prescribed by their doctor, and take their blood pressure regularly.

-Diabetics should control their fasting blood glucose level.

-Cancer patients, immunosuppressed, with HIV, should take their treatments and keep track of their disease.

González affirmed that in Panama more than 40% of the population is overweight, a comorbidity that can become a risk factor.

“The issue of prevention is extremely important, since if a patient has good control of his disease, he will be able to get out of this episode of COVID-19,” he said.

Finally, he added that diet plays a fundamental role, since there are certain foods that contain vitamins and minerals, which help strengthen people’s immune system.

END NOTES

Thankfulness can be defined in many ways but is solely dependent upon your individual perspective.  That perspective is ever-changing.  What you are thankful for today, may not be your top priority tomorrow; it’s all about psychological focus.  Those of strong moral direction, will most usually recite the same structure, (deity, family, friends, work…), but wont necessarily recall on a moment’s notice the same secondary points from one time to the next.

Right now, staring at this screen, I’m thankful that I have internet, electricity and a working laptop with it’s bells and whistles.  I look to my left and see the Spiderman figurine that my grandson left here by mistake over a month ago; I’m thankful that he’s in my life even under this limited capacity.

Last night was Thanksgiving for us gringos.  During our meal, each one of us recited what we were grateful for, what thanks we wished to give.  We all gave a proper response, but Brett Jr later came to me and said, “I’m thankful that Mom got enough whipped cream for all of us to share.”  So, in his honor, I share with you this image of how he probably wished he could have eaten his Pumpkin Pie last night:

Stay safe…  Stay healthy…  And by all means (unless you’re out getting more Cool Whip to recreate this image) STAY HOME!!…
